Accusys 8405d60797 Fix 5W1H+: max_tokens 2048->4096, skip empty summaries

- max_tokens was too low, truncating LLM JSON output
- Added guard to skip storing empty parent_summary
- Applied fix to all 3 entry points (analyze, batch, pipeline)
